Xanthelasma looks like a yellow colour excess soft skin that appears over the eyelid near the nose. It is derived from ancient Greece, where \u201cxanthos\u201d means yellow and \u201celesma\u201d means plate. Palpebrarum is a Latin word that means \u201cnear or related to the eyelid. Xanthomas are cholesterol-rich depositions that can appear anywhere in the body during various disease states. XP is a type of xanthoma that occurs over the eyelids. It is said to be associated with several medical conditions like hyperlipidemia, diabetes, and thyroid dysfunctions. It may be a predictor of ischemic heart disease, myocardial infarction, or systemic atherosclerosis. Though associated with lifestyle diseases it usually does not cause major problems. People visit surgeons <\/strong><\/a>because it becomes a\u00a0cosmetic\u00a0concern.<\/p>\n

Epidemiology<\/h2>\n

Xanthelasma palpebrarum is an uncommon skin lesion in the general population with a crude incidence of 1.2%. It is more common in women than in men. The age of onset ranges from 20 to 70 years, but it is most commonly seen between the ages of 35 and 55 years<\/p>\n

Pathophysiology<\/h2>\n

Xanthelasmas are the most widely recognized type of xanthoma. They are yellowish deposits composed of a cholesterol-filled substance that appear mostly over the medial aspects of the eyelids. Several medical conditions are associated with the appearance of xanthelasma, including hyperlipidemia, diabetes, and thyroid dysfunctions. It might appear in patients with normal lipid profiles, namely, cholesterol and triglyceride levels<\/p>\n

History\u00a0and\u00a0Physical<\/h2>\n

These lesions are often seen in the fourth and fifth decades of life. Once xanthelasma has developed, it will not spontaneously disappear but will remain the same or increase in size. The majority of people come to attention because of cosmetic concerns. Typically, xanthelasma palpebrarum<\/a> patient presents with soft, yellowish papules and plaques over the medial canthus of the upper lid. Sometimes it might be solid, semi-solid, or firm. These papules cannot be squeezed. These deposits usually\u00a0\u00a0will not go away on their own and typically will grow up with the time<\/p>\n

Based on their appearance they have been divided into 4 types<\/h3>\n

  • Grade\u00a0I\u00a0<\/strong>\u2013 were\u00a0limited to the\u00a0upper\u00a0eyelid<\/li>\n
  • Grade\u00a0II<\/strong>\u00a0\u2013 involved\u00a0the\u00a0medial\u00a0canthus,<\/li>\n
  • Grade\u00a0III<\/strong>\u00a0\u2013 affected\u00a0both\u00a0eyelids\u00a0on\u00a0the\u00a0medial\u00a0side,\u00a0and<\/li>\n
  • Grade\u00a0IV<\/strong>\u00a0\u2013 involved\u00a0both\u00a0medial\u00a0and\u00a0lateral\u00a0sides\u00a0of\u00a0the\u00a0upper\u00a0and\u00a0lower\u00a0eyelids<\/li>\n<\/ul>\n

    Treatment\u00a0\u2013<\/h2>\n

    Medical management involves lifestyle modifications such as regular physical\u00a0exercise\u00a0and low-fat\u00a0diet in addition\u00a0to lipid-lowering drugs.<\/p>\n

    1. Chemical\u00a0peel\u00a0like\u00a0TCA.\u00a050\u00a0%,<\/li>\n
    2. CRYOTHREPAY,<\/li>\n
    3. CAUTERY,<\/li>\n
    4. BLADE,<\/li>\n

    6. Radiofrequency\u00a0machines\u00a0can\u00a0be\u00a0used\u00a0to\u00a0treat\u00a0some\u00a0cases\u00a0of\u00a0xanthelasma because it is a very safe method compared to other modalities, but it\u00a0is\u00a0less effective, and it\u00a0might\u00a0be\u00a0expensive.<\/li>\n<\/ol>\n

      Disadvantages\u00a0of\u00a0other\u00a0method-<\/h2>\n

      Surgery\u00a0around\u00a0the\u00a0eyelids\u00a0is\u00a0fraught\u00a0with\u00a0complications\u00a0and\u00a0can\u00a0result\u00a0in\u00a0ectropion,\u00a0eyelid retraction, and injury to the eye itself. The use of cryotherapy and chemical\u00a0cauterization\u00a0can\u00a0lead to\u00a0severe\u00a0scarring\u00a0and skin\u00a0discoloration<\/p>\n

      Can it recur again?<\/h2>\n

      Xanthelesma tends to recur if not removed completely or properly. In certain cases even after proper removal and uncontrolled cholesterol level it may recur with an interval of 12-18 months.<\/p>\n
